I have had years of experience as a Medical Social Worker who is specifically trained to work with patients who are elderly or chronically ill. For more than two decades, I have worked as a Medical Social Worker in a variety of settings including longterm healthcare facilities, dialysis units, a Health Maintenance Organization, and an Adult Day Health Program. I have designed and implemented a full range of programs to more adequately support the residents and their families. At a large Health Maintenance Organization, I worked as a Case Management Social Worker and: - Co-Developed a very successful pilot program entitled, "Caring For Your Aged Loved One"
- Worked as a Case Manager advocating for homebound elders.
- Assesed the safety level of residential environments so that elders were able to stay in the own homes for a maximun period of time.
My extensive experience has provided me with the knowledge others desperately need in order to make well-reasoned decisions, particularly when confronting end of life issues. As a Medical Social Worker, I have: - Advocated for clients and supervised volunteers.
- Implemented the safe return of patients to the community,
making sure that the necessary infrastructure was in place - Facilitated individual and group sessions with clients
and their families. - Worked on the Alzheimer's HelpLine to offer support and education
to families caring for loved ones with dementia - Serve as Hospice Volunteer since 2000
- Facilitate monthly Huntington's Disease support groups for
caregivers, patients, and persons at-risk
