I have had years of experience as a Medical Social Worker who is specifically trained to work with patients who are elderly or chronically ill. For more than two decades, I have worked as a Medical Social Worker in a variety of settings including longterm healthcare facilities, dialysis units, a Health Maintenance Organization, and an Adult Day Health Program. I have designed and implemented a full range of programs to more adequately support the residents and their families. At a large Health Maintenance Organization, I worked as a Case Management Social Worker and:

  • Co-Developed a very successful pilot program entitled, "Caring For Your Aged Loved One"
  • Worked as a Case Manager advocating for homebound elders.
  • Assesed the safety level of residential environments so that elders were able to stay in the own homes for a maximun period of time.

My extensive experience has provided me with the knowledge others desperately need in order to make well-reasoned decisions, particularly when confronting end of life issues. As a Medical Social Worker, I have:

  •  Advocated for clients and supervised volunteers.
  •  Implemented the safe return of patients to the community,
        making sure that the necessary infrastructure was in place
  •  Facilitated individual and group sessions with clients
        and their families.
  •  Worked on the Alzheimer's HelpLine to offer support and education
        to families caring for loved ones with dementia
  •  Serve as Hospice Volunteer since 2000
  •  Facilitate monthly Huntington's Disease support groups for
        caregivers, patients, and persons at-risk

Caryl Diengott holds a Bachelor of Arts in Sociology/Social Work from Suffolk University and a Master's Degree in Social Work with a concentration in Gerontology from Boston College. As the Director of Social Work in a Home Care Agency, she has facilitated hundreds of team and family meetings, constantly emphasizing the individual needs of the clients.
